Thank you for choosing to register for the North Shore TMA’s Green to Work program.

Please click on the link below to download the Green to Work Registration Form and monthly tracking log.  If you plan to participate in the incentive program, enter your choice for the dates of your 3-(consecutive) month incentive program on the line provided.

You must start your Green to Work incentive program on the first of the month.  Your incentive program will continue for 3 consecutive months.  At the end of your 3-month incentive period, you must submit your Green to Work calendar (available online for download) by the 5th of the following month.  After submitting your incentive program log, you will receive an e-mail from the TMA with your program statistics and requesting that you choose a gift card (from the online gift card list) in the amount of your Green to Work award (based on the total number of alternative commutes you logged during the 3 month incentive program).

If you cannot commit to commuting at least 24 times using an alternative commute mode during a 3-month period or if you have previously completed the incentive program and wish to participate in the monthly/quarterly raffle, please check the Yes box next to Monthly Raffle.

If you enter Yes next to monthly raffle, each time you log your commute at least once during the month and submit your commute log by the 5th of the following month, you will be entered into the random monthly raffle. Log a minimum of one alternative commute at least once each month and submit your log on time for 3 consecutive months in a calendar year and you are eligible for the Green to Work $100 raffle prize. The $100 raffle prize will be awarded quarterly (January/April/July/October).
 

Logging your commute is easy and will take less than 30 seconds each day!

Please complete the registration, save the form to your hard drive using the following format: 
YourName.Company.REGISTRATION.Startdate (e.g. AlMarrone.Philips.REGISTRATION.3-1-11).
